I'm not sure where you get the 80% of cash rate figure, but
having recently gone through some very extensive surgery and
seen the bills, I can say from personal experience the
discount to insurance companies is closer to 40% (and
sometimes more) off the cash rate - putting it much closer to

Not a chance. A commercial insurance contract that pays 60%
of charges is seen as generous in today's market.
Where I work (as a surgeon), our collections average about
35% of charges, almost completely due to contractual write-
offs with payors rather than billing or collection problems
per se. This is with a mix of commercial, Medicare, and
Medicaid patients. We basically make money on commercial
patients, break even on Medicare, and lose our shirts on
Medicaid, but we feel it is our civic duty to not turn away
