I think the point is that the Doctor/Insurance system is a closed circuit. The
more money that flows round the circuit, the better.
As soon as you start paying an outside body (the gym industry) money is flowing
out of the system. That's bad for the system ...

First understand we don't have a healthcare system in the
US, we have a medical delivery system. Doctors receive
compensation not for fixing you, but for treating you - thus
the incentives are around delivering care. In addition, the
standard is often "what is customary" - only recently did we
really start looking at what is effective (I know it sounds
weird, but it's true). Also doctors and others are paid by
treatment, NOT cure. Thus no disincentive to keep costs
down.
In addition, the insurance normally pays the doctor, and was
normally paid by the employer - you (the patient) are really
a 3rd party. That also messes up the various incentives.
Overall we've ended up with a system that is the highest
cost, and, per the WHO statistics, one of the worst in the
world.
