Maybe this is the programmer in me.
I understand "forced" to mean that she was
compelled by outside forces, possibly against
her will.

I can see that her circumstances make it sensible
to take that road. But if she were wealthy there's
no reason she would sign up for Medicare Part B and
no compulsion to do so. Any penalties (as I understand it)
only kick in if one tries to sign up at some later date.

In contrast, the ACA penalty/tax is compelled
and therefore "forced" in an explicit way.
