for a simple appendectomy at your local hospital? How much
for a kidney transplant? How does that compare with the next
town over?

A couple more assumptions about free markets:
Substitutes are available. If wheat costs too much, use barley or rye. There's
no substitute for medical care when it's required.
Switching costs are negligible: Don't like buying your wheat from grain broker
Smith? Get it from broker Jones. That doesn't work for healthcare where one
establishes trust-based relationships with a primary care physician.
Health care does not operate in any way approximating the market model and the
assumption that market-based solutions is inappropriate.
