And that goes back to the refusal to hire UNIX programmers in the 80s.
All of their programmers seem to be nothing but click monkeys that don't
understand the foundations...Even moreso their "administrators".
A "trade school" learning type of thing, though some trade schools can

They don't seem to do step 3 very thoroughly, and instead
expect the customer to do much of the testing for them.
And, they don't seem to have done steps 4 and 5 enough
times.
Oh, and the "work it out in your head" part of step 1 was
rather weak..
Step 2 (regardless of media) they certainly do, and do, and
do, til the result is a big bloat-ware mass of software
carnage.
