Right now VistA is actually Public Domain (large parts of
it), because it was developed BY THE DOCTORS AND NURSES of
the VA (remember that open source rule about scratching an
itch?). A group of folks have taken that and actually
turned it into Open Source and continue developing it.

WorldVistA EHR VOE/ 1.0, the only open source EHR that meets
Certification Commission for Healthcare Information
Technology (CCHIT) ambulatory electronic health record (EHR)
criteria for 2006. WorldVistA EHR VOE/ 1.0is based on and
compatible with the U.S. Department of Veterans Affairs (VA)
world renowned EHR, VistA(R). "
VA Wins Harvard's Prestigious Innovations Award - "This
Award is sponsored by the Ash Institute for Democratic
Governance and Innovation at Harvard University’s Kennedy
School of Government and is administered in
partnership with the Council for Excellence in Government."
The link to the latter press release is dead, but I remember
the article - a Harvard award for excellence in Health Care
- from the standpoint of the doctors and nurses (NOT the IT
community).
"Long in the tooth" in software is a term I often see
associated with "it works and they keep using it, so we
can't sell them new stuff." I know companies running 30
year old binaries - because they keep working. Why replace?
VistA unfortunately doesn't get much play in the non-
Government world - because health care companies keep
looking to commercial businesses to support them, and all
the "health care IT" businesses sell very expensive, very
proprietary, very non-userfriendly software, with very
expensive maintenance contracts, locked-in data formats, and
very restrictive EULAs. For some reason no business seems
to have developed like Red Hat around WorldVistA. Maybe an
opportunity for an Open Source entrepreneur?
