I remember certain BBSes that required one to call them, login, and be
automatically hang up. They called you back to login, automatically hung up on
you once they did so. You called them back, logged in, and this time, if
everything went correctly, you stayed connected. If it didn't, you started the
cycle again, an hour or so later.
If your system couldn't answer the callback, or you were not at the callback
number, tough luck. You were not going to be allowed to login to the system.
If the magic phrase here is "out of band", I do recall a BBS that sent
a text message, that needed to be input, prior to proceeding beyond the login
page. I don't remember its name or phone number, though. :(
Either _The Red Book_, or its predecessor had a section on sending text messages
using a shell script. It also had a section on login authentication, and
